Course Information 

Program Includes:

6 hours of self-study content
2 hours of live (remote) case discussion with CU School of Pharmacy faculty experts
40 hours of practice/experiential training 4 hours/week over an 8-week timeframe

Program Overview

The focus of the program is on pattern management for complex diabetes patients.  This program is designed to prepare pharmacists with the knowledge, skills, and practical training to use pattern management and technology to provide patient centered diabetes care.  The program provides comprehensive didactic instruction on key concepts for managing complex diabetes patients using current standards of practice and technology as well as practical training where pharmacists apply concepts in actual pharmacy practice settings. 

Completing the Program - The program is broken up into three components. Participants must complete all three program components in order to gain the certificate of completion for the program.

Part 1 - Self-Study - By Monday November 6th Complete 12 modules (30 minutes each - total of 6 hours) of asynchronous/self-study materials through the online learning classroom. Material covers:

Part 2 - Live (Remote) Case Discussion On Wednesday November 8th  Participants will join our hosts and PharmD diabetes experts at the scheduled live case discussion by Zoom.

Part 3 - Practical Training/Experiential - Complete 40 hours of practical training with CU School of Pharmacy tele-health partner RxVIP.  Participants will be contact to set up their start date with experiential partner.

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This activity is accredited as a certificate program and provides 48.0 contact hours of practice-based continuing pharmacy education (6.0 hours home-study, 42.0 hours live).  

Successful completion of the self-study component involves passing the assessment within each of the 12 self-study modules with a grade of 70% or higher and will result in 6 contact hours of CPE credit (0.6 CEUs).  

Successful completion of the live session discussion component involves attending the full 2-hour live seminar. Successful completion of this component will result in 2 contact hours of CPE credit (0.2 CEU).

Successful completion of the practical/experiential training session component involves completing the 40 hours of orientation and patient care (documentation will be provided by RxVIP confirming hours) and receive a grade of 70% or higher on the assessment evaluation completed by RxVIP.  Successful completion of this component will result in 40 contact hours of CPE credit (4.0 CEU).

Participants must complete all components listed above, and CLAIM credit for each component to receive the Certificate of Achievement for this program.  

This educational public service program (CPE component) offers Continuing Education credits only; it does not offer credit towards any degree program at the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences. The target audience for this activity is pharmacists.

Additional Options in the Advanced Diabetes Education Series

This registration page is for the full Advanced Diabetes CE Certificate Program. If interested in just the home study and live case discussion portions of the certificate, participants are welcome to register for one of the two options below and exclude the experiential portion. **Please note, a certificate will not be award if the experiential portion is not completed.**
