Course Information 

Home Study

The focus of the home study modules is on pattern management for complex diabetes patients. It is designed to prepare pharmacists with the knowledge, skills, and practical training to use pattern management and technology to provide patient centered diabetes care.  Home study modules provides comprehensive didactic instruction on key concepts for managing complex diabetes patients using current standards of practice and technology.

Part 1 - Self-Study - By Monday October 24th  Complete 12 modules (30 minutes each - total of 6 hours) of asynchronous/self-study materials through the online learning classroom. Material covers:

Part 2 - Live (Remote) Case Discussion on Wednesday October 26th - Complete 2 hours of live session virtual patient case discussions with CU Pharmacy Faculty.
